Yuriy Alayev, PA-C

Yuriy is a Certified Physician Assistant in Phoenix, Arizona.
He graduated from Midwestern University in 2006, earning a degree in Medical Science and Physician Assistant studies with Cum Laude.
Yuriy has extensive experience in family medicine, internal medicine, urgent care, emergency medicine and orthopedics. He has worked in the valley’s most busy emergency rooms and urgent care centers for over 20 years.
Yuriy is well-versed in diagnosing and treating common medical conditions in the comfort of your home. He is dedicated to providing you with excellent customer service and meeting all your healthcare needs.
Diana Suleymanova, MSN-PH, APRN, FNP-C

Diana Suleymanova is a board-certified Family Nurse Practitioner (FNP-C) through the American Association of Nurse Practitioners with experience in urgent care, acute care, home health, and chronic disease management. She earned multiple degrees from Grand Canyon University, including her Master of Science in Nursing and Family Nurse Practitioner training. Diana has cared for patients in medical-surgical units, long-term care facilities, rehabilitation settings, and fast-paced clinical environments, giving her extensive experience evaluating and treating a wide variety of medical conditions.At YA Medical, Diana provides mobile urgent care and in-home medical services throughout the Phoenix area with a strong focus on compassionate, patient-centered care. She has a special interest in infectious diseases, preventative medicine, and helping patients receive convenient medical care in the comfort of home.
Glen McCracken, MD, MBA
Collaborative Medical Director
Dr. McCracken is a graduate of Creighton University School of Medicine.
He is an Emergency Room Physician that has won “Top Doctor” award in Phoenix for several years. He has also served as a President and CEO of Scottsdale Emergency Associates Ltd.
Dr. McCracken continues to practice Emergency Medicine in Arizona and serves as a collaborative Medical Director for YA Medical LLC.
Charlene Baldwin, MSN, FNP-C

Charlene Baldwin, MSN, FNP-C, is an experienced Family Nurse Practitioner with an extensive background in urgent care, primary care, emergency medicine, and mobile and long-term care. She provides comprehensive care to patients of all ages and has broad experience treating acute and chronic conditions, performing minor procedures, IV therapy, diagnostic testing, and patient education. Charlene brings decades of nursing and advanced-practice experience to YA Medical, with a focus on delivering knowledgeable, compassionate, and convenient care to patients in the comfort of their homes
Tamara Mavasheva, RN, BSN
Tamara Mavasheva, RN, BSN, is an experienced Registered Nurse with more than 18 years of healthcare experience in medical-surgical nursing, endoscopy, case management, and patient care coordination. Throughout her career, Tamara has developed extensive experience in patient assessment, IV therapy, medication administration, care coordination, and patient education. At YA Medical, she brings her clinical expertise and compassionate approach to providing convenient, high-quality care to patients in the comfort of their homes.
